Tra i micronutrienti che sono stati oggetto di studio in rapporto con l'autismo un posto di rilievo spetta alla vitamina D. Essendo il numero di studi abbastanza rilevante e' stato possibile fare una metanalisi, come risulta dal seguente lavoro di cui riporto l'abstract.

Serum concentration of 25-hydroxyvitamin D in autism spectrum disorder: a systematic review and meta-analysis.

Abstract

Vitamin D may play an important role in the etiology of Autism Spectrum Disorders (ASD). Vitamin D is regarded as a neuroactive steroid affecting brain development and function. It plays an essential role in myelination, which is important for connectivity in the brain. Studies have shown that decreased vitamin D levels in patients, decreased maternal vitamin D levels during pregnancy, and decreased exposure to solar UVB might increase the risk for ASD. In addition, autism symptoms and global functioning may improve after vitamin D supplementation. Here, we sought to aggregate information from previous publications on vitamin D levels and ASD, in order to achieve a higher statistical power and thereby to determine the validity of vitamin D deficiency as a risk factor for ASD. For this meta-analysis, 11 studies met the inclusion and exclusion criteria, accounting for a total of 870 ASD patients and 782 healthy controls. Levels of serum 25(OH) D in participants with ASD were significantly lower than controls, suggesting that lower vitamin D level might be a risk factor for ASD.

La carenza di vitamina D sarebbe un fattore di rischio per autismo in gravidanza e un fattore di peggioramento dei sintomi nel bambino affetto. Degno di nota il fatto che vi sarebbe una correlazione tra entita' del deficit e gravita' dei sintomi. The mean 25-OHD levels in patients with severe autism were significantly lower than those in patients with mild/moderate autism. Serum 25-OHD levels had significant negative correlations with Childhood Autism Rating Scale (CARS) scores. Ancora  piu' interessante il fatto che con la correzione del deficit vitaminico si e' avuto un miglioramento dei sintomi Collectively, 80.72% (67/83) of subjects who received vitamin D3 treatment had significantly improved outcome, which was mainly in the sections of the CARS and aberrant behavior checklist subscales that measure behavior, stereotypy, eye contact, and attention span.

La metanalisi fatta dal gruppo di autori cinesi e danesi é una buona premessa per fare sperimentazioni sull’uso della vitamina D come terapia sintomatica dell’autismo. L’invito implicito nella metanalisi é stato colto da un gruppo neozelandese che ha pubblicato un disegno di sperimentazione randomizzata controllata con vitamina D e placebo, aggiungendo anche un braccio con omega3 e un altro braccio con l’associazione di vitamina D e omega3. Questo per sfruttare il presunto sinergismo tra le due sostanze

Prima dell’arruolamento nella sperimentazione a tutti i bambini, che devono avere, oltre alla diagnosi di disturbo dello spettro autistico, un’etá compresa tra i 2 e mezzo e gli otto anni, viene dosata la vitamina D (serum 25(OH)D) nel sangue. La vitaminemia D deve essere inferiore a 75 + 10 nmol/L per i bambini che vengono inclusi nello studio in inverno e inferiore a 105 nmol/L + 10 nmol/L per quelli che vengono inclusi durante l’estate. Questo per non rischiare l’ipervitaminosi.
Nel caso l’esame mostri una carenza conclamata di vitamina D, prima di entrare nello studio la carenza viene corretta e solo dopo la correzione il bambino puo’ essere incluso nella sperimentazione.
Si prevede come durata della sperimentazione un anno e un numero minimo di partecipanti per ogni braccio sperimentale di 42, in modo che i risultati abbiano valore statistico.
Le premesse biologiche ci sono, il disegno sperimentale pare buono. Speriamo che gli autori riescano a portare a termine la sperimentazione e che pubblichino i risultati comunque, siano essi positivi, come si spera, o negativi.
